WVCA is pleased to launch a new funding opportunity for voluntary and community organisations in Wolverhampton, with grants of up to £5,000 available.
The Hospital Discharge – Social Prescriptions Grant Programme is designed to support activities that help people transition from hospital back to life at home—particularly those at risk of loneliness, isolation, or poor recovery outcomes.
Research shows that social support and meaningful connections play a vital role in recovery. By embedding social prescribing into the discharge process, this programme aims to:

Improve patients’ short-term quality of life

Support their medium-term recovery and well-being
-
Reduce long-term hospital readmissions
Examples of eligible activities include:

Low-impact exercise programmes (e.g. chair-based mobility)

Befriending support (in person or by phone)
-
Transport assistance (hospital pickups, appointment buddies)

Counselling or emotional well-being support
We are also particularly keen to support projects that address identified gaps, including support for:

Males aged 18–30
-
Adults aged 25–50
-
People with ADHD or low-level mental health needs

Evening and weekend service availability
Eligibility criteria:

You must be a voluntary or community organisation with a community bank account
-
Projects must be based in Wolverhampton and benefit local residents

All funds must be spent by 30 September 2025

You must accept referrals from the Social Prescribing service and submit a final report with case studies
Applications will be reviewed on a rolling basis and assessed by a project panel. Scoring will be based on clarity, credibility, and how well your proposal meets programme outcomes. Please apply early—funding is limited and will close once fully allocated.
