WVCA is pleased to announce the extension of our funding programme designed to support voluntary and community organisations in helping patients transition back to life at home after an extended hospital stay.

Returning home can be a daunting time – physically, emotionally, and socially. Without the right support, many patients face loneliness, isolation, and an increased risk of readmission. That’s where you come in.

Grants of up to £5,000 are available for projects that embed Social Prescribing and community support into the hospital discharge process. This could include activities like:

✅ Gentle exercise sessions
✅ Peer support groups
✅ Skills-building workshops
✅ Community engagement activities that help people rebuild confidence and connections

Research shows that increasing social connections after discharge can significantly improve recovery and reduce strain on healthcare services. By funding these initiatives, we aim to improve quality of life in the short term, boost recovery in the medium term, and cut hospital readmissions in the long term.
