Hospital Discharge: Social Prescriptions Grant Programme

WVCA is pleased to announce the extension of our funding programme designed to support voluntary and community organisations in helping patients transition back to life at home after an extended hospital stay. Grants of up to £5,000 are available for projects that embed Social Prescribing and community support into the hospital discharge process.
